The main distinction between a comforter and a duvet is that a comforter is a single piece of bedding, whereas a duvet requires two separate pieces — an insert and a cover. A comforter is typically quilted with the filling distributed evenly, whereas a duvet has an insert that serves as the fill. You can still use a duvet as a regular comforter, but some duvet comforters are not as easy to wash and should be covered with a duvet to keep it clean.
A duvet comforter is intended to be used as an insert, so while it can be used as a comforter without the cover, some people prefer the comfort and softness that a regular comforter provides.
A duvet is a plain, usually white comforter filled with down or a down alternative like polyester fill. It is intended to be used inside a duvet cover. A duvet cover is the outer layer of a duvet comforter that is usually more decorative than the inner comforter.
To wash a comforter, place it in a washing machine alone on a super load. Move to dryer promptly when done. Check comforter when dryer finishes making sure it's dry and avoid mildew.
